• Important Information

    I understand the payments are due on the FIRST of each month as stated in the Parent Handbook unless otherwise discussed with the program director. I further understand should any payment or financial questions arise, I must speak with the director about them and that the teacher has no authority to discuss any financial matters with me or on my behalf with the director.

    Registration fee: all ages (non-refundable): $250

    Curriculum Fee: (non-refundable; Pre-k 3 & 4 only): $75

    Late Fee: $ 25 (assessed on the 8th day of the month)

    Returned payment fee: $25

    Late Pickup Fee: $5 per minute after 2:05pm

    Payments are to be made online using our payment processor. All families must have an authorized auto payment on file. Iunderstand that the center hours are Monday - Thursday 9am - 2pm and Friday 9am - 12pm. We are a school year only program and do not offer summer programs.

    | understand if I need to drop my child from the program for any reason I must give a written 2 week notice to the director and understand full payment for the two weeks must be made regardless of my child's attendance during those two weeks.

    I understand that my child may be dropped from the program if he/she is absent for more than 30 days (without notice Please refer to the Parent Handbook for more information on this.

    I certify that all information I have given on the forms provided is true and correct to my knowledge. have read and agree to the regulations listed here and within the Parent Handbook I have been given.

  • I will allow The Way Church PDO & Preschool to apply the following non-prescription items to the child listed above. It is agreed that I will be informed of any of the above as soon as possible if used. Diaper rash ointment or cream First aid cream

  • By signing this form, I agree to allow The Way Church Parent's Day Out and Preschool staff to administer CPR if needed and/or secure emergency medical treatment for my child. This form also releases The Way Church Parent's Day Outand Preschool Program and persons as stated above from any liability from any accident or injury which may occur regarding the above.

  • On occasion, The Way Church PDO and Preschool takes photographs or makes an audio or videotape recording of children and/or adults involved in church activities and/or PDO and Preschool activities, and such images or videos may be used by the staff to memorialize school activities. In addition, such images and recordings may be used in The Way Church PDO and Preschool publications, websites, social media and advertising materials to let others know about our ministry. If local news organizations hear of our activities or events, our church may invite or allow them to photograph or record our events for news reporting on special interest features. I consent to the use of any such audio/visual recording or photograph of my child or me to be used, distributed or displayed in an appropriate manner as deemed suitable by the church and/or directors of the PDO and Preschool program. This consent includes, but is not limited to: photographs, videotape, and audio recordings. Furthermore, I give my permission for my child to be interviewed by the news media, or for such images listed above to be used by the news media.
